NATIVE VALVE ENDOCARDITIS

Twenty-one patients with native valve endocarditis caused by coagulase-negative staphylococci were studied; 14 had pre-existing valvular or congenital heart disease. Although commonly subacute in presentation, complications of endocarditis were frequent: arterial emboli, new electrocardiographic conduction system abnormalities, congestive heart failure, annular or myocardial abscesses and disruption of valve leaflets. Cures were achieved in 10 of 12 patients who were treated medically and 7 of 9 who were treated surgically. Microbiologic studies of 16 coagulase-negative staphylococci from patients with endocarditis were made; only 8 were identified as Staphylococcus epidermitis. All isolates were susceptible to vancomycin. Antibiotic resistance was usually associated with nosocomial acquisition of endocarditis. Rather than representing contamination, coagulase-negative staphylococci in blood cultures may indicate life-threatening endocarditis. However, if we pay attention to the selection of antibiotics for therapy and to the occurrence of heart failure due to intracardiac complications, treatment of this form of endocarditis will be successful. Organisms must always be tested for cryptic resistance to beta-lactam antibiotics. Valve replacement may be required frequently.

Se estudiaron 21 pacientes con endocarditis valvular nativa causada por estafilococos coagulasa-negativos; 14 tenían cardiopatía valvular o congénita preexistente. Aunque con frecuencia subaguda en la presentación, las complicaciones de la endocarditis fueron frecuentes: embolia arterial, nuevas anomalías del sistema de conducción electrocardiográfica, insuficiencia cardíaca congestiva, abscesos anulares o miocárdicos y alteración de los folíolos valvulares. Curas se lograron en 10 de 12 pacientes que fueron tratados médicamente y 7 de 9 que fueron tratados quirúrgicamente. Se realizaron estudios microbiológicos de 16 estafilococos coagulasa negativos de pacientes con endocarditis; Sólo 8 fueron identificados como Staphylococcus epidermitis. Todos los aislamientos eran susceptibles a la vancomicina. La resistencia a los antibióticos usualmente se asoció con la adquisición nosocomial de endocarditis. En lugar de representar la contaminación, los estafilococos coagulasa negativos en hemocultivos pueden indicar una endocarditis potencialmente mortal. Sin embargo, si prestamos atención a la selección de antibióticos para la terapia ya la aparición de insuficiencia cardíaca debido a complicaciones intracardíacas, el tratamiento de esta forma de endocarditis tendrá éxito. Los organismos siempre deben someterse a pruebas de resistencia críptica a los antibióticos beta-lactámicos. El reemplazo de la válvula puede ser necesario con frecuencia.

EARLY GASTRIC CANCER

Surgical specimens from gastric resections for carcinoma, obtained between 1977 and 1979, were studied and compared with resections from a pre-endoscopic period between 1967 and 1969. The incidence and clinicopathologic features of early gastric cancer were analyzed. It was found that early gastric cancer now comprises significantly more lesions leading to gastric resection, because endoscopy and biopsy of gastric ulcers of benign appearance are much more often used. Recently, there were 6 cancers in a total of 22 resection specimens; in the pre-endoscopy period, there was only one in 27. Five of the 6 patients in the recent period are alive free of disease 5 years after surgical resection. The single patient in the earlier period died postoperatively. There were three elevated or polypoid lesions, a single flat lesion and three depressed or ulcerated lesions which were interpreted as benign peptic ulcers on conventional upper gastrointestinal studies. Findings on endoscopic biopsies were positive in all cases (6 of 6). Early gastric cancer is highly curable by surgical resection. It must be recognized more frequently. A high index of suspicion on the part of physicians is essential for success.
